Student Registration

To register a new student(s), parents/guardians should:
 
  • Complete all forms below. (Registration forms can also be picked up in the middle school office.)
  • Return all completed forms to the office or email at [email protected]
  • Include ALL required documents with your packet. (birth certificate, proof of residency, shot records, medical forms, etc.)
